This template was what I made for diaper card for a baby shower (please see my gallery for how it turned out). I don't have the exact name for each part of the template - I tried to leave notes here and there, hope it's helpful. ;) The type of paper being used to make this card will help make it looks better compare to if I'd have used regular paperstock - Have fun!
